'Eye is very suitable for gene therapy'

GENETIC BLINDNESS

It's a long time before we find a cure for genetic blindness, but a  beginning has been made. University College of London has found vision improvements in three out of eight clinical trial subjects treated with gene therapy. They were suffering from a rare inherited eye disease, Leber CongenitalAmaurosis (LCA).  James Bainbridge, eye surgeon, Moorfields Eye Hospital and UCL Institute of Opthalmology, tells Jayashree Nandi about gene therapy in ophthalmology:

  • How is gene therapy used to tackle blindness?
In cases of inherited blindness or poor vision. it's due to the lack of a critical gene that detects light. Our research is to replace that mutant gene with a normal working copy. We have designed a way by which the new gene can be placed in the cells.  A disabled virus is taken to package the gene.  The disabled virus carries it into the cells and releasees it at a spot behind the retina.  A micro-surgical technique is used for this. 

  • What are the recent developments after the first human trial?
Our current research involves trials on individuals with LCA. It is caused by defects or mutation of the gene protein, RPE 65. The first result of the trials is that it pays off, there is improvement in night vision and there are no side-effects. However, it's too early to know how long the improved vision will remain.
         
                We have done eight trials on patients between the ages of 10 and 20 years.  Three have shown considerable improvement in night vision. Again, it's too early to know. Not all trial subjects have responded. For instance, the first subject to undergo the trial, Robert Johnson, did not respond to the treatment.

  • What are the shortcomings  in this large - scale treatment?
Gene therapy in ophthalmology is in the clinical trial phase. There could be possible side effects, there could be damage to the retina, the virus that delivers the gene could lead to inflammation of the eye and there could be unpredictable effects.  We need to investigate a lot more.

               However, the eye is a lot more suitable for gene therapy. It is the first organ to benefit from the trials.It is because the organ is small and contained, the disabled virus is likely to remain in the tissue for some time instead of spreading across.

  •     Is there a timeline when this therapy can be commercialised?
The changes in each trial subject take place after several months.  We are monitoring them over several years.  Even though we have seen encouraging results in our institute and other groups in the US have also validated it, it will take time to see what long -term impacts it will have.

HISTORY REPEATS ITSELF: Abhishek Bachchan

It's happened, again!
          Why is it that when one producer decides to make a film on some historical figure,  the rest of Bollywood also jumps onto the bandwagon ? This happened last in 2002, when poor Bhagat Singh's spirit was revived in not two, but four films, with Ajay Devgn and Bobby Deol respectively playing the daring hero of the uprising!

               Now it's revolutionary Surya Sen's turn. The man, known as Masterda, who led the Chittagong Uprising of 1930, has two films being made on him --one by LA filmmaker Bedabrata Pain; and the second by Ashutosh Gowariker, Manoj Bajpayee plays Surya in Bedabrata's film Cittagong and Abishek Bachchan plays him in Ashutosh's Khelein Hum Jee Jaan Sey. While Ashu's film is being made on a grand budget, Bedabrata's will be against a more conservative expense sheet. Ashu's film is also based on Manini Chatterjee's Bengali award-winning novel Do or Die while Bedabrata's is on history itself. And both the films will release this year !

Purab debuted in Bas Yun Hi that starred Azhagi Nandita Das and his first big break came with My Brother Nikhil that was directed by Onir. Purab has just come on board the I am... series, to be directed by Onir. He will be a part of the film Afia with Nandita, who incidentally has lost oodles of weight (not that she was ever plump) and is looking awesome. Now, isn't that enough reason for Purab to rejoice ?

"ASIN PATIENTLY LISTENED TO MY NARRATION OF A FILM. SHE SAID SHE WOULD GET BACK TO ME. I AM STILL WAITING FOR HER CALL"

     Asin is in a catch-22 situation. The actress is insistent on doing only Bollywood films with 'top saleable male stars' and thus refusing offers from Tamil cinema. After this London Dreams fiasco, offers up north have dried up for Asin and the latest is that she is going about town talking about pursuing her first love painting. Meanwhile, in Kollwood, Asin is still the most-wanted herine in every big actor's wish-list.

Kollywood grapevine is abuzz that at least four big projects are being lined up with her in mind, The strongest of the rumours is that Asin has confirmed that she will star in the Tamil remake of Siddique's Malayalam flick Bodyguard with Ilayathalapathy Vijay, Asin will play the female lead in the film (which had Nayanthara in the original) and shoot will dcmmence in March end.

Another project awaiting her approval is dirctor A R Murgadoss's upcoming film with Suriya for producer Udhayanidhi Stalin, which will start rolling in May-June. Meanwhile, rumour mills have been working overtime that Simbu met Asin in Mumbai and discussed his dream project Vaaliban. Also, Jayam Raja-who introduced Asin to Tamil films-has approached her for his next film with Vijay. And, the biggest of them all is that director Gautham Menon is in talks with her for his Hindi remake of Kaakha Kaakha.

Asin, when contacted, refused to talk about her future moves and cited the usual 'no dates' clause for not doing Bollywood films. Source close to her say that though she's open to doing Tamil films if the roles are good, she will take her own time to decide. Says a hit director, who has worked with Asin, "I called her up recently and she patiently listened to my narration of a film that I'm planning to do with a Tamil superstar. She said that she would get back to me within two days, but I'm still waiting for that call."

CANCER FACTS

Sometimes too much information is a goo thing. This is true especially in the case of cancer

Cancer is a disease characterized by excessive reproduction of cells, which serves no useful purpose, and is not compensated by appropriate cell loss. Cancer cells have certain unique attributes, which distinguish them from normal body cells-they originate from one single cell, which means all the cells in a given cancer have the same genes. They can grow indefinitely, are genetically unstable and have the ability to spread to distant organs (metastatise),This ability to disseminate is the hallmark of cancer, and distinuishes it from benign tumours. Metastatic spread denotes stage IV or advanced disease and is usually incurable. Metastasis may occur through the blood, lymphatics or by direct spread. The liver, lungs, bone and brain all of which have a rich blood supply are commonly affected by metastases.

The cause of cancer may be due to external factors like chemicals, radiation or viral infections, or internal due to hormones, decreased immunity or genetic changes, which may be hereditary. Cancer causation or carcinogenesis is a complex process. The time between exposures or mutations to detectable cancer is often ten or more years ! When a person comes with a tumour to the doctor, it may have already been present several years without causing any symptoms.

In India, cancer is a major problem with 8,00,000 new cases of cancer being detected every year, Though cancer often shows up late, there ae certain warning signs which should not be ignored - unexplained weight loss, loss of appetite, lump in the breast or bleeding from the nipple recent change of bowel habits, bleeding after menopause, change in shape/size or bleeding from a mole and chronic cough. Cancer can be detected at a very early stage, bu doing a screening or early detection of asymptomatic or unrecognised  disease by the application of acceptable, inexpensive tests or examinations.

There are several modalities used to treat cancer - surgery, radiation, therapy, chemotherapy, endocrine and biological therapy. The type of treatment depends on the type of cancer, location, stage and general condition of the patient. When cancer has metastasised, it is usually beyond the realm of cure, except in a minority of cancers, and treatment is with chemotherapy.

     For more than two years now, Beena Madhavan and her sisters have dedicated themselves to serving one of the noblest causes that society is faced with today the fight against cancer. Beena and her sisters - Bindu Dijendranath , Biju Jayavevan and Brinda Das - have formed a team that has been aptly christened B-force, dedicated to bring about cancer awareness and aid in the psychological and monetary support of cancer patients.The sisters have create quite a name for themselves with their admirable efforts. "B-force was established in September 2007 with the sole aim of catering to the medical concerns of policemen and their families," explains Beena Madhavan, while emphasising the role that her cancer support group plays in society, today. "My father, the late.

      K Mohandas, the former DGP of CID, Tamil Nadu  was a true policeman in every sense of the word and this group was established by us sisters, in his loving memory, for the people he served with - the Police. When my mother died of cancer in 2001, we knew that this was the cause we would take up, " she says.

The most admirable trait that B-force possesses in addition to the number of cancer awareness programmes, conducted and the regular check-ups organised by the trust is the fact that a great deal of financial assistance is also provided by B-force to patients who are unable to afford expensive treatment. "W e have five trustees with us as part of B-force who have stood by us and are of immense support in our ventures, " says Beena, "There was this case where a policeman needed Rs. 13,000 every month to pay for medicines following a heart transplant while his monthly income was a mere Rs. 9,000. B-force has been supporting him by taking care of his expenses incurred for medicines. "On a final note, Beena Madhavan expresses hope that cancer will find a cure someday. "It's important to be aware, " she says, "Regular check-ups and the right kind of treatment in case of a suspected case of cancer can nip the illness in the bud."

     When he narrated the story of the film to me, I was hooked! I play a 17-year-old girl in this beautiful love story. What attracted me to the project is that it has the creme de la creme of Kollywood Prabhu Deva, Kalpathi sir, Jayam Ravi, Harris Jayaraj and Nirav Shah. It's a dream team."

     The shoot of the film, set in france, will begin by the end of April on the french Riveria. Harris and Prabhu deva, after their musical sitting in Australia, have come out with two tunes, Prabhu is planning to shoot 80 percent of the film in France, and the remaining 20 percent in Chennai.
